HHM Health Enrollment

At HHM Health, all individuals must enroll as a patient in order to receive our services regardless of whether they are uninsured or have Medicare, Medicaid, or private insurance. The enrollment process, which typically takes 20 minutes, is meant to benefit our patients, as it allows them to receive prescriptions and healthcare on a sliding scale fee based federal poverty guidelines based on income and family size. Following the enrollment process, individuals will walk away knowing what payment slide they were placed on and be eligible for all HHM Health services.

How to Enroll

Visit the Enrollment Center on the third floor of 5750 Pineland Drive. Bring all the required individuals and documents. All information provided must be current, dated within the last 30 days. Assistance is readily available, including multiple translators.

Preregister to enroll using the link below. After filling out the online form, you will be contacted by HHM Health within two business days to gather additional information and schedule your first visit. If you are not contacted within 2 business days, please call 214-221-0855. 

Once you are enrolled as a patient of HHM Health, our team can help you enroll in Medicaid or Texas Healthy Women if you are eligible.

Enrollment Requirements

1. Proof of Household Income from everyone in the household who works family and non-relatives:

  • Most recent paycheck stubs, (please bring at least 4 paycheck stubs) (Must be dated within 45 days of registration) OR
  • Previous year tax return, OR
  • Employer statement of income, which states gross income and frequency of pay. This letter must be dated, signed and include a telephone number. (The Employment Form must be filled out by your employer. Written letters will not be accepted. You can download the form below.)

2. Birth Certificate (if available)

3. Proof of Residence, which includes apartment lease, mortgage documents, or utility bills (electricity, water, gas or cable). (if available)

4. Social Security Card (If applicable)

5. Government Assistance Award Letter (if applicable to you or anyone in your household):

  • Food Stamps
  • Child Support
  • Social Security/ Disability
  • Unemployment
  • SSI
  • Public Housing
  • TANF

6. Valid Picture ID and Insurance Card (if any)

Yes, all individuals must be enrolled in order for them to be a patient at HHM Health.

No, enrollment does not provide you with insurance. Enrollment is meant to get you set up on our payment assistance program. However, the enrollment center will help patients of HHM Health sign up for Medicaid if they are eligible.

No, one application is required per person.

Children do not need to be present. However, all individuals 18 or older must be present during the enrollment process.
